POSITION SUMMARY: The Clinical Pharmacist provides comprehensive pharmaceutical care to optimize medication therapy outcomes and ensure the safe, effective, and cost-efficient use of medications. This role involves collaborating closely with physicians, nurses, and other healthcare professionals to develop and implement patient-centered medication plans, monitor therapeutic responses, and provide evidence-based recommendations. The Clinical Pharmacist also participates in formulary management, medication-use evaluations, quality improvement initiatives, and staff/patient education. ESSENTIAL RESPONSIBILITIES/DUTIES: Conduct comprehensive medication therapy reviews for patients on specialty medications, ensuring safety, efficacy, and adherence. Provide disease-specific counseling and education to enhance patient understanding and engagement in therapy. Partner with prescribers, nurses, case managers, and payers to facilitate initiation and continuity of specialty therapies.
